Our Story
Annandale Health & Community Services is a 501(c)(3) non-profit corporation organized and existing under the laws of the State of Minnesota. Any person who is at least 18 years old and who makes a cash donation specifically for the purpose of membership of $25.00 or more per calendar year shall be a Member of the Corporation from the date of receipt of the donation through March 15th of the following year. All members of record as of December 31st are eligible to vote in the election held in the month of March of the following year. Current employees of Annandale Health & Community Services, or any affiliate or subsidiary corporation, are not eligible for membership. Former employees are eligible for membership one year after the end of their employment. Notification of this membership fee shall be sent in February, of each year to all Members on record as of the 31st of December of the preceding year.
Annandale Care Center, a 45-bed skilled nursing facility, opened its’ doors in 1982. In 1984 the Care Center added an ancillary service wing to provide space for physical, occupational and speech therapy inpatient and outpatient services and also added a garage. In 1991 the garage was converted to a chapel and two detached garages were built. In 2004 Annandale Care Center went through an extensive remodeling including changing double occupancy rooms to single rooms and adding private rooms.
Centennial Villa was built in 1987 to provide senior housing to independent persons 55 years of age or older. This building is connected to the Annandale Care Center by a link. Meals can be purchased for a fee and light weekly housekeeping is provided. 22 apartments consisting of eight two bedroom, two large one bedroom and twelve one bedroom apartments are available. Due to an overwhelming response for assisted living services, these apartments also offer assisted living.
Centennial Villa Assisted Living, 23 apartments; three two bedroom, nine large one bedroom and eleven standard one bedroom apartments are available for assisted living.
In July of 2008 Centennial Villa opened three Memory Care cottages in a separate building connected to the Care Center. These three cottages house up to 12 people and replaced the memory care apartments within the assisted living building. The cottages are designed as a household with separate bedrooms with private bathrooms.
Annandale Health & Wellness opened in 2009. The Wellness Center offers state of the art exercise equipment, a therapeutic swimming pool, hot tub and an Exercise Studio. Along with providing additional benefits for individuals receiving physical therapy, it is available for membership for those 50 or better.
In 2021 Pleasant Vista opened serving the 55 plus community with independent senior living with option to add assisted living services.
